Medicaid coverage program for low income elderly, blind, or disabled persons. Individuals that may need long-term services and supports, also known as long-term care (nursing home or home- and community-based care), he/she may be eligible for HUSKY C.

Eligibility: Age 65+ or disabled; Must be U.S. citizen or have permanent resident status; Must have source of income; Net income must be below standard of living established by the CT State Legislature; Asset limit for individual is $1600; Asset limit for couple is $2400; Income limits vary depending on where in Connecticut you live and the coverage you need (HUSKY C)

Application Process: Applicants can apply online to DSS at www.connect.ct.gov, in person by going to his/her local DSS office, or by mail -You can also use the ‘W-1E’ form to apply for these programs
Documents Required: Proof of identity; Proof of income; Proof of liquid assets; Proof of address; Proof of citizenship or green card; Proof of age if age 65 or over; Proof of disability if under age 65; Proof of housing expenses
