Program Hours:
Disability Access Center Hours: Monday through Friday, 8 am - 5 pm
Description: Located near the intersection of Bay Laurel Way and Moraga Way, between Sacramento Hall and Kadema Hall.

Program Delivery
Eligibility: Any Sacramento State enrolled student who meets the standard of a qualified individual with a disability (QID), regardless of whether temporarily or permanently disabled.
Languages: Contact for additional language information, English
Application Process: Apply for services through the online DAC Student Portal. Answer the short questionnaire and upload the required documents. Once the application is received and the account is activated, an appointment with an Access Coordinator will need to be made. Visit the website for additional details.

- Developmental: Student Medical Provider Form
- Learning: most recent learning disability assessment/evaluation
- Physical/Medical: Student Medical Provider Form
- Psychological: Student Medical Provider Form
