Sutter SeniorCare PACE (Program of All-Inclusive Care for the Elderly) is a health plan designed exclusively for frail older adults in Sacramento County.  Provides coordinated medical, social, and emotional support to help participants age in place safely in their own homes and communities, while avoiding or delaying nursing home placement and reducing hospital visits.

Program Delivery
Eligibility: Serves low-income seniors and veterans. Participants must meet the following criteria:

• Be at least 55 years old;

• Must live in Sacramento County;

• Must be eligible for nursing home level of care as determined by the State of California (must have a chronic condition that needs to be managed);

• Must live at home or in a community setting without jeopardizing their health or safety.
Languages: Translation services are available for multiple languages on request. American Sign Language Interpreter available on request, English
