Description
Assists eligible individuals with performing daily tasks safely in their own home. Services include: 
  • Feeding
  • Bathing
  • Dressing
  • Housekeeping
  • Accompanying individuals to medical appointments 
  • Paramedical services
  • Protective supervision
IHSS recipients are responsible for finding their own home care providers. Also responsible for firing if services are not performed according to client's specifications. Program Delivery
Eligibility: California resident age 65 or older or blind or disabled, live in his/her own home, has a Medi-Cal eligibility determination; and complete submit a Health Care Certification Form from their physicians.
Languages: Interpreter services available, English
Application Process: Complete online application can be submitted via mail, fax, or in person. Have physician complete the Health Care Certification Form. After a home visit by Social Worker. Client will be notified if they qualify via letter.
Program Fees: No fee
Documents Required: Health Care Certification form [SOC 873] completed by a Licensed Health Care Professional.
