If a child already has CCS medical benefits and is in need of a Medical Therapy Program (MPT)

Eligibility: -Open to Shasta County children and youth under age 21
-Must have CCS eligible medical condition
-Must meet family income requirements (generally under $40,000 a year with exceptions)
Languages: Language interpretative services available upon request, American Sign Language, Spanish, English
Application Process: Referral may come from parent, school, family physician, hospital; Fill out a CCS application online or call
