Program Delivery
Eligibility: Sutter patients 6 months and older. Vaccinations are free and available regardless of immigration status.

- The updated Pfizer and Moderna vaccines for 2023-2024 are recommended for everyone 6 months and older. Alternatively, the updated Novavax vaccine for 2023-2024 may be given to persons 12 years of age and older.

- ​​​Infants and toddlers 6 months to 4 years of age who are unvaccinated are recommended for 2-3 updated doses. Those who have received prior doses are recommended for 1-2 additional doses.​

- Everyone 5 years of age and older is recommended for a single updated dose as long as it's been at least 2 months since the last dose.

- People 12 years of age and older who are not vaccinated should get 1-2 doses of the updated Pfizer, updated Moderna, or updated Novavax COVID-19 vaccine.

- ​People with weakened immune systems may receive additional updated doses. It is suggested to talk to one's healthcare provider to see if additional doses are recommended.

Application Process: By appointment only. Schedule appointment online or call to schedule. Booking online, sign in, or enroll in My Health Online to schedule a first dose appointment. Choose "COVID-19 Vaccination - First Dose: Sutter Clinic." Answer questions to find a location.

Program Fees: No fee.
Documents Required: Patients under 18 must have the consent of a parent or guardian to receive the vaccine. Parent or guardian must be present at vaccine appointment or have signed the consent form here: https://www.sutterhealth.org/pdf/for-patients/minor-patient-consent-covid-19-vaccine.pdf

For each additional dose after the first, bring your COVID-19 Vaccine Record (paper or digital copy). To obtain a digital copy of your vaccine record, visit myvaccinerecord.cdph.ca.gov.
