Eligibility: Child must be under 21 years old with an eligible medical diagnosis, financially and residentially eligible

Provides medical therapy services that are delivered at public schools throughout San Bernardino County
Languages: Spanish, English
Application Process: Anyone can self refer, contact is via telephone and mail
Application packet is mailed after referral is made
Payment methods: No fee, Accepts Medi-Cal
Documents Required: Proof of physical residency in San Bernardino County, verification of current Medi-Cal or financial documentation and medical records verifying eligible medical condition
