The Health & Wellness Program at Chapa-De can help you reach your goals for a healthy life. This team is specially trained to support patients to eat healthy, stay active, and manage conditions like diabetes and obesity. Chapa-De invite you to learn more about your health and choices that can help you feel better. Services are available one-on-one and/or as part of a group. This program welcomes all Chapa-De patients who receive care in our Medical Department.
