Chapa-De's Diabetes Program invites participants in their group classes and education programs that focus on the prevention and management of diabetes. As a department, it is their mission to help patients establish health goals and provide you with tools and support people's need to accomplish those goals. One can take charge of your health and make improvements for you and your family. Making just a few changes can lead to big results.

- Dietitians
- Nurse Case Managers
- Lifestyle Coaches who provide information, support, and tips on eating healthy, exercising, and improving overall health and wellbeing
Chapa De also provide a variety of wellness programs, classes, and support groups throughout the year. Many services and groups are available virtually during the COVID-19 pandemic.
