Provides a free prediabetes risk assessment online. Encourages one to complete the assessment if they respond with "Yes" to any of these questions:
  1. Are you overweight?
  2. Are you over 40 years old?
  3. Do you have a family history of diabetes?
  4. Do you have high blood sugar?
  5. Do you have low or no physical activity daily?
  6. Do you have high blood pressure or elevated cholesterol?
  7. Are you American Indian, Hispanic Latino, African American, or Asian American?
  8. ​Do you have a previous diagnosis of gestational diabetes?
Please note: This assessment is for those who do not currently have a diagnosis of Type 1 or Type 2 diabetes.
