Documents Required:

Proof of Insurance if Medi-Cal, Medi-Cal Managed Care or Blue Shield PPO/EPO eligible. Copy of the immunization record. Minor children under the age of 18 must be accompanied by a parent or guardian. If a minor and wanting an Human Papilloma Virus (HPV) vaccination, parent/guardian does not need to be present.
