Provides health care services to children and youth in foster care. Offers medical, dental, mental, and developmental needs services. Includes the following services:

  • Medical and healthcare case planning

  • Health and Education Passport (HEP)

  • Helps foster caregivers get comprehensive health assessments and dental exams

  • Expedites referrals for medical, dental, mental health, and developmental services

  • Coordinates health services for children in out-of-county and out-of-state placements

  • Provides medical education through the interpretation of medical reports

  • Training for foster team members. Trains on the special health care needs of children and youth in foster care.

Services offered in consultation and collaboration with social workers and probation officers.
