Screening Questions for Eligibility

1. Are you Self-Identified Vulnerable (SIV) or Medical-Baseline?

• • Must be one of these or they are not eligible.

2. Do you live in a single family, multi family, or mobile/manufactured home?

• • Single family is okay. More than 4 units is not eligible. Manufactured/mobile homes, not eligible.

3. Do you own or rent?

• • If renter please have landlord contact information ready.

4. Do you have a permanently installed backup battery storage installed like a Tesla Powerwall?

• • If yes, not eligible.

5. Does your home currently have a permanently installed generator that powers your whole home automatically when there is an outage?

• • If yes, not eligible.

6. Do you know what type of panel you have (brand)?

• • If Zinsco, Sylvania, or Federal Pacific not eligible.

7. Is there a generator or generator connection point on your property?

• • Would need to be willing to have the connection point removed to be eligible.
