Description
Provides Enhanced Care Management, which is comprehensive care management from a single lead care manager who coordinates all health and health-related care, including physical, mental, and dental care, and social services. Also coordinates client transportation for health care appointments.
Adult populations:
- Adults At Risk for Avoidable Hospital or Emergency Department (ED) Utilization
- Individuals and Families Experiencing Homelessness
- Serious Mental Illness (SMI)/Substance Use Disorder (SUD)
- Individuals Transitioning from Incarceration
- Adults Living in the Community and At Risk for Long-Term Care Institutionalization
- Nursing Facility Residents Who Want to Transition to the Community
Children & youth populations:
- Youth At Risk for Avoidable Hospital or Emergency Department (ED) Utilization
- Children (up to Age 21) Experiencing Homelessness
- Transitioning from Incarceration
- Enrolled in California Children's Services (CCS)/ CCS Whole Child Model with Additional needs Beyond the CCS Eligible Condition
- Involved in, or with a History of Involvement in, Child Welfare Services/Programs (Including Foster Care up to Age 26)
- Experiencing Serious Emotional Disturbance or identified to be at Clinical High Risk for Psychosis or Experiencing a First Episode of Psychosis
