Description
Home Health Services provides physician-directed in-home health care services for individuals who are homebound and recovering from illness or injury, managing chronic medical conditions, or requiring rehabilitation services. Services are designed to support recovery and independence in the home setting and may include skilled nursing, home health aide services, physical therapy, occupational therapy, speech therapy, and medical social work. Home Health staff coordinate with the client's physician to develop an individualized plan of care.

​Eligibility: Individuals of any age who are homebound and:
- Require additional treatment following illness or injury
- Have chronic medical conditions requiring ongoing assessment
- Need assistance with personal care due to mobility, vision, or cognitive limitations
- Have a newborn or child with special needs requiring skilled home health support
