Program Delivery
Eligibility: Must be:
-SSI/SSP or Medi–Cal eligible
-Blind, disabled or 65 years of age or older living in a home, apartment or residence of choice (not including a hospital, nursing home, assisted living or licensed care facility) and unable to live safely at home without care
Languages: Spanish, English
Application Process: Individuals who already have Medi-cal, can call or visit to submit an application or fax it. Once application is received, a social worker will go to home to conduct an assessment of your mental and physical abilities and limitations to determine whether applicant qualifies.
Program Fees: Depending on income, some recipients must pay a Medi-Cal share of cost
