Eligibility:

Chronically homeless and disabled youth ages 18 - 25, seniors aged 55 and up, and families with at least one disabled parent or child.

Entry is conducted through the county-wide Coordinated Entry System (CES). In order to be prioritized and placed on the By-Name List (community-wide waitlist) for CES-related housing services, individuals/families must complete a Vulnerability Index-Service Prioritization Decision Assistance Tool (VI-SPDAT) assessment. Once on the list, individuals/families will be prioritized and matched with housing interventions based on vulnerability and program eligibility.
VI-SPDAT assessments are conducted through a variety of community service providers for their existing program participants or community-wide through the 2-1-1 Coordinated Entry System Line, Seven days a week, 24 hours a day. Please note, housing providers and 2-1-1 staff are not able to directly accept or make placements as matching to housing programs is done exclusively by the CES system.
