Provides medical care, behavioral health, diabetes services and recovery services for patients with Medi-cal/Partnership Healthplan insurance. Telehealth appointments available as needed. 

Medical care services:
  • Primary care
  • Urgent care (for minor emergencies)
Behavioral health services (for all ages):
  • Individual counseling
  • Family counseling 
Substance use recovery services:
  • Counseling
  • Nutrition services
  • Medication-assisted treatment (with buprenorphine)
  • Treatment for Opioid addiction
  • Treatment for alcohol use disorder
