Description
Provides State-authorized medical marijuana identification cards for those with serious medical conditions. These cards are available to people whose physicians have recommended the use of marijuana. Participation by patients and primary caregivers in the MMICP is voluntary.

Offers a registry database for verification of qualified patients and their primary caregivers. The registry allows law enforcement and the public to verify the validity of a qualified patient or primary caregiver's MMICP. Provides authorization to possess, grow, transport, and/or use medical marijuana within California.

Eligibility: Must be recommended by a physician; patient may designate a primary caregiver and apply for a caregiver identification card; must apply within county of residence
Languages: Language interpretative services available upon request, American Sign Language, English
Application Process: Call for appointment and application form; All applicants (patient and primary caregiver) must be present during the application process
Payment methods: Fees vary, Accepts Medi-Cal
Program Fees: $100 for patient and primary caregiver; with Medi-Cal, $50 for patient and primary caregiver; with CMSP $0
Documents Required: Doctor’s recommendation, photo ID/DL, proof of residency
