Eligibility: Must have a significant medical illness or treatment such as organ transplantation, diabetes, chemotherapy, asthma, cystic fibrosis, kidney disease, seizures and other chronic illnesses
Languages: ASL interpreter available, Spanish, English
Application Process: Appointment required
Payment methods: Fees vary, Accepts Medi-Cal, Accepts Medicare, Accepts most insurance

Does not accept straight Medi-Cal or Molina
