Statewide office of Protective Services for the Elderly investigates reports of known or suspected physical or mental abuse, exploitation, or neglect of adults ages 60+ living in the community; Also investigates reports about abuse/neglect cases of individuals, ages 60+ living in long term care facility settings.

A fillable W-675 PDF form can also be utilized for reporting. Please FAX the W-675 form to 860-424-5091, in addition, a dedicated inbox psereferrals.dss@ct.gov has been created to receive the form, also. Individuals can find the form at the Department of Social Services web link below. Please note: Telephone reports are preferred.

Program Delivery
Eligibility: Age 60 years and older individuals experiencing physical, mental and emotional abuse, neglect (including self-neglect), abandonment and/or financial abuse and exploitation
Application Process: To make a report, call Protective Services for the Elderly at the Department of Social Services during business hours at the toll-free line: 1-888-385-4225. After business hours or on weekends or state holidays, call 2-1-1; If calling from outside Connecticut, call 2-1-1, 24/7 at 1-800-203-1234
Program Fees: No fee
Documents Required: A fillable PDF Report Form (W-675) is also available, as an option.; FAX the W-675 form to 860-424-5091; However, telephone reports are preferred

Unpublished Notes211 Call Specialists can talk to the PSE worker about financial emergencies that the elder is dealing with, to see if the PSE worker can access funds to help the elder. In very rare circumstances, after assessment, PSE may be able to help with financial crisis in order to safeguard the elder until they are stabilized. HOWEVER, this assistance is a last resort, and does not take the place of the 211 Call Specialist exploring options from the traditional assistance that may be available to that elder. (foundations, town social service depts., social service agencies, etc.) BOTTOM LINE: Do not refer elders who have unpaid bills to PSE! Referrals to PSE are for elders who are being (or at risk of being) abused, exploited, or neglected.
