Description
Connecticut Home Care Program for Elders manages medical and non-medical support services that are needed by frail individuals ages 65+ to avoid institutionalization. Managed services include visiting nurse services, home health aide, chore assistance, homemakers, adult day care, home delivered meals, companion services, respite care, transportation, emergency response systems, and other services necessary to support independent living. Cost of services cannot exceed cost of institutional care. Purpose of the program is to keep frail elders from being institutionalized. There are no income limits for the program, but cost of care is calculated according to a sliding fee scale. Asset limits depend upon income level. The benefit of program participation for eligible seniors who must pay for full cost of care is that they can receive the case management and care oversight services that CT Home Care Program case managers provide. The PCA (Personal Care Assistant) Program is for disabled individuals whose PCA services ended when they reached age 65.

Program Delivery
Eligibility: Connecticut resident, ages 65+; No income limit; There are asset limits; Must be U.S. citizen or have permanent resident status; Must be in need of short or long term nursing home care or at risk of institutionalization; Cost of services must be less than cost of institutional care
Application Process: Alternate Care Unit
Program Fees: Sliding fee; Fees range from no fee to full fee for services needed
Documents Required: FOR STATE-FUNDED COMPONENT OF THIS PROGRAM, self-declaration suffices for most areas of the application EXCEPT proof of citizenship or legal resident status
FOR MEDICAID and MEDICAID WAIVER-FUNDED COMPONENT OF THIS PROGRAM THE FOLLOWING APPLIES: Proof of address (current rent receipt or mortgage bill, utility bill); Proof of income (if employed: copies of pay stubs; if self-employed: tax returns or bookkeeping records; copies of checks from source of income; an award letter or signed statement from the person or source of any income); Proof of all assets (bank books, bank statements, trust fund agreements, copies of stocks/bonds/U.S. Savings bonds, life insurance policies, a letter from a financial institution, a copy of a car registration, deeds or legal agreements); Proof of citizenship (self-statement or letter of citizenship if naturalized) or proof of legal resident status (green card, Visa papers); Proof of age (birth certificate, baptismal certificate, immigration papers, passport, voter registration card, drivers license, hospital records, adoption papers, naturalization certificate, census records, school records, insurance company records, social security records, physician's statement, statements from disinterested third party); Proof of family size (birth certificates or rental lease); Social Security numbers for all family members (Social Security cards); Proof of disability (Social Security determination letter or physician's statement); Proof of housing expenses (lease or mortgage bill, utility bills); Proof of medical expenses (medical bills); If employed, proof of employment (pay stubs or letter from employer); If applicable: documentation of trusts, transfer of assets, burial funds, reverse mortgage
