Description
Acquired Brain Injury Medicaid Waiver Program, a special Medicaid waiver program for adults who have an acquired brain injury pays for medical and non-medical services such as case management, personal care assistance, chore services, respite care, vocational supports, housing supports, and other services as needed to keep the individual living in the community.

Program Delivery
Eligibility: --Adults ages 18-64 who meet the functional requirements of the program
--Income under 300% of the maximum monthly SSI benefit
-- Must meet "Level of Care Requirement" which means that without the waiver services the individual would have to be placed in a long term care facility
--Must be able to participate in the development of a Service Plan that offers a community alternative to institutional living
--An adult determined eligible for the ABI Program qualifies for all Medicaid covered services
Application Process: Applicants or their representatives may contact the Community Options Unit for instructions
on how to be placed on the waiting list; Individuals will be contacted when a space is available;
Applicants who wish to apply for the ABI II waiver program must submit the Acquired Brain
Injury (ABI) Waiver Request Form (W-1130) to:
