Description
Waiver program enables severely disabled individuals to be cared for at home, or includes those that are institutionalized and/or at risk of institutionalization, and be eligible for Medicaid based on the individual's income alone. Without the waiver, the income of legally liable relatives is counted when the individual is cared for at home. There is a long waiting list for this program; however, families of severely disabled individuals who want to care for their family member at home are encouraged to add their name to the waiting list.

Program Delivery
Eligibility: Severely disabled individuals age 21 or younger; Individual's income must be at or below 300% of SSI; Asset limit is $1000; Legally liable relative's income is waived
Application Process: DSS maintains a waiting list for those interested in gaining access to the program when an opening becomes available; To be placed on the waiting list, the applicant or representative should contact the Community Options Unit
Program Fees: No fee
Documents Required: Proof of income and assets; Katie Beckett Model Waiver Assessment/Plan of Care and Individual Ed Plan/Planning and Placement Team Meeting minutes (IEP/PPT) if applicable
