Severely disabled individuals age 21 or younger; Individual's income must be at or below 300% of SSI; Asset limit is $1000; Legally liable relative's income is waived

DSS maintains a waiting list for those interested in gaining access to the program when an opening becomes available; To be placed on the waiting list, the applicant or representative should contact the Community Options Unit
