Description
Money Follows the Person is a program for people who have lived in nursing, chronic care, or intermediate care facilities for at least 3 months and who are on or eligible for Medicaid. The MFP program provides payment for services that augment participation in other community based support programs such as CT Home Care Program for Elders, PCA waivers, and DDS or DMHAS programs.

Program Delivery
Eligibility: Eligible for Medicaid; Must have been living in an institutional facility for at least 3 months
Application Process: Apply through the Money Follows the Person unit at DSS; once screened, the Agencies on Aging and Independent Living Centers will manage the transition into the community for those deemed eligible for the program
