Description
Housing Engagement and Support Services program, is a new initiative to cover supportive housing benefits under Medicaid by combining both Medicaid services and non-Medicaid housing subsidies to eligible individuals at risk for homelessness. CHESS will provide assistance in locating and securing stable housing, assist with healthcare and substance use needs, non-Medical transportation, and increased access to other community supports. The initiative combines the Coordinated Access Network and housing subsidy programs administered by the Connecticut Department of Housing; supportive housing programs administered by the Connecticut Department of Mental Health and Addition Services; and the Money Follows the Person program administered by the Connecticut Department of Social Services (the state agency administering Medicaid).

Program Delivery
Eligibility: Ages 18 or older, have active Medicaid, and meet all the requirements for CHESS which include being at risk of homelessness, have a Modified Charlson Comorbidity Index score of more than 4, have a behavioral health diagnosis recognized by the International Classification of Diseases, and have at least 2 critical needs
