Documents Required:	VITA REQUIREMENTS Copy of last year's taxes (if available) Valid photo ID for all adults on the return Social security cards or ITIN letters for everyone on the return Wage and earning statements: W-2, W-2G, 1099-R, 1099-MISC, 1099-NEC Health insurance forms: 1095-A or 3895 Medi-cal forms: 1095-B or 1095-C Form 1098-T (if you were a student) Interest and divided statements: 1099-INT, 1099-DIV Total amounts paid to childcare providers and their tax ID numbers Bank routing and account number if you want to receive a direct deposit
