Application Process:	Apply online or call 1. Create an account with RADx-UP data collection system 2. Get 3 FREE at-home COVID test 3. Submit your results (+/-) You will receive $20 gift card a few weeks after submitting your test results and completing all of the surveys in the "My Current Forms" tab 4. Complete additional surveys for a second gift.
