 Description
Provides transportation to medical appointments for people on Medical Assistance, and some pregnant women and children on MinnesotaCare who have no other transportation available to them.

Door to door paratransit service may not be available in all cases. Type of service depends on the insurance provider of the client. Some insurance providers may limit transportation to within certain mileage.

If transportation isn't available, gas reimbursement or bus tokens may be provided. Call for case-specific information.

Program Delivery
Eligibility: Hennepin County residents who are enrolled in Medical Assistance, a managed health care plan such as HealthPartners, Hennepin Health, Medica or UCare, or some pregnant women and children enrolled in MinnesotaCare
Languages: Interpreters available
Application Process: - Call; clients on a health plan must call their health plan such as HealthPartners, Hennepin Health, Medica or UCare
