Description
Comprehensive health and wellness services to address health concerns, provide treatment including medications, coordinate health care services, provide health education and coordinate access to health and social services in the community. Services are provided through health clinics at emergency shelters and drop-in centers in the downtown and surrounding area.

Also provides a Respite Program for homeless adults recovering from illness or injury or who have recently been released from the hospital. Respite services are provided through the Salvation Army - Harbor Light agency in Minneapolis.

Program Delivery
Eligibility: - Health Care: Any homeless adult or child living on the streets, in emergency shelter or transitional housing, with friends or relatives or who have been homeless within the past year; serves Hennepin County

- Respite Program: Any individual who meets Hennepin County shelter eligibility requirements who is living outside or on the streets, in a shelter or transitional housing facility or with friends or relatives AND is homeless, recovering from an acute medical problem, such as frostbite, fractures, pneumonia or recent hospitalization, does not require a skilled-nursing facility or 24-hour supervision, can perform daily living activities, is medically and psychiatrically stable, can behave appropriately, is compliant with shelter rules and is willing to cooperate with respite staff

Application Process: - Health Care: Walk-in; appointments are not required; recommend to call ahead for clinic sites and hours

Program Fees: Sliding scale; accepts most health plans; clinic services will not be denied based on inability to pay
