Public Assistance applications taken for cash, health care, food support and Emergency Assistance programs at the following community-based sites:

Service Center Locations:

- South Minneapolis Human Services Center and Mental Health Center: 2215 East Lake Street, Minneapolis; 8 am - 4:30 pm Monday - Friday by appointment or walk-in. Full range of health and human services; replaces the Century Plaza location. Child care available on 2nd floor.

- Health Services Building Human Services Center: 525 Portland Avenue South, Minneapolis; 8 am - 4:30 pm Monday - Friday by appointment or walk-in. Limited drop-in child care is available.

- Northwest Family Human Services Center: (co-located with Community Emergency Assistance Program (CEAP) and Osseo Area Schools), 7051 Brooklyn Boulevard, Brooklyn Center 55429. 8 am – 4:30 pm Monday - Friday by appointment or walk-in. Limited drop-in child care is available.

- VEAP Community Human Services Center - (co-located with Volunteers Enlisted to Assist People - VEAP) 9600 Aldrich Avenue South, Bloomington 55420, 8 am - 4:30 pm Monday, Tuesday, Wednesday and Friday, 8:30 am - 6:30 pm Thursday, walk-in. Limited drop-in child care is available.

- Hopkins/Wells Fargo Building Human Services Center: (co-located with Resource West): 1011 1st Street South, Suite 109, Wells Fargo Building, Hopkins 55343. 8 am - 4:30 pm Monday - Friday; Thursday by appointment.

- North Minneapolis Human Services Center: 1001 Plymouth Avenue North, Minneapolis. 8 am - 4:30 pm Monday - Friday, by appointment or walk-in 

Satellite Center Locations:

- Little Earth Satellite Center: 2438 18th Avenue South, Minneapolis 55404. Monday - Friday by appointment. Note: American Indian community only.

- IOCP Satellite Center: 1605 County Road 101 North, Plymouth 55447 Monday - Friday by appointment. Phone 612-543-0006 or 612-596-0777.

- Office of Multi-Cultural Services/Powderhorn Partners Building Satellite Center: 1201 East Lake Street, Minneapolis 55407. By appointment only.

- Call for Human Services or Satellite Center appointment (Satellite Centers are by appointment only). Do not enter a case number. Hold for an operator.
