Program Delivery
Eligibility: Hennepin County residents who have a diagnosis of serious and persistent mental illness (SPMI) and receive services through either a Community Service Provider (CSP) or a Hennepin County case manager
Languages: Interpreters available
Application Process: Program services can only be accessed by a Community Service Provider or a Hennepin County case manager who makes the request on behalf of the person with a serious and persistent mental illness

Documents Required: Verification of serious and persistent mental illness by a mental health professional
