Program Delivery
Eligibility: Families of children, birth through age 3, who are at risk for developmental delays; serves Hennepin County residents
Languages: Interpreters available
Application Process: - Call; staff will mail out forms to be filled out with a return envelope

- Participation is voluntary; participants can leave the program and re-enter at any time

- Children receiving Early Childhood Special Education services do not qualify for the Follow Along Program as it would be a duplication of services
