Description
A web-based application that is comprehensive and integrates assessment and support planning for people who need long-term services and supports in Minnesota. The assessment takes a person-centered approach to ensure services meet each individual's strengths, goals, preferences, and assessed needs. The assessment replaces the following assessment tools:
  • Developmental Disability Screening
  • Long-term Care Consultation Assessment
  • Personal Care Assistance Assessment
  • Home Care Nursing Assessment (to be included in a future enhancement)
The assessment determines eligibility for publicly funded programs and services including:
  • Alternative Care Program
  • Brain Injury Waiver
  • Community Alternative Care Waiver
  • Community Access for Disability Inclusion Waiver
  • Consumer Support Grant
  • Developmental Disability Waiver
  • Elderly Waiver
  • Essential Community Supports for Seniors
  • Moving Home Minnesota
  • Personal care assistance
  • Case management
  • Semi-Independent living services
