Program Delivery
Eligibility: - 16 years of age or older in Hennepin County
- Have no minor children in client's care
- Must have strong ties to Hennepin County or experiencing homelessness in Hennepin County
- Are sleeping outside or in a place not meant for human habitation
- Household income does not exceed 30% of the Area Median Income (AMI) for the household’s size
- Single adults staying at a homeless shelter in Hennepin County for over 14 days will be identified by shelter staff for an assessment
Languages: Interpreters available
Application Process: - Email the Coordinated Entry Assessors or visit one of the walk-in locations listed on the Hennepin County Coordinated Entry website https://www.hennepin.us/residents/human-services/coordinated-entry

- Emails will be responded to within 72 hours

- An appointment will be scheduled to complete the assessment; can meet clients where they are located
