HEALTH NEEDS ASSESSMENTS: Conducts health needs assessments by the program's registered nurses. Nurses will then consult with the individual, other staff, and community professionals about the health needs of specific customers.
IN-HOME PERSONAL CARE: Offers services to adults 18 and older with functional disabilities allowing to remain in the home and maintain independence when possible. A range of in-home personal care services and therapeutic services such as emotional support and self-care training.
