Description

ASSISTED LIVING HOUSING: Provides housing to adults who need supervision and assistance with activities of daily living and who are unable to live by themselves. Project Home providers offer care in their own homes to one to four residents who have a disability.

Eligibility
|
Adults age 18 or older who are mentally and/or physically disabled. The adult cannot live independently and has a disability that can be managed in a home environment.
A provider can be someone 21 years of age or older who is living in the
home where care will be provided and has the willingness to assist an adult
with a disability to continue living in the community. A provider must have
sufficient income
