Description
|
CASE MANAGEMENT: Assists eligible persons to remain independent and living in their community for as long as possible. Case management activities include assessment, referral and continuous evaluation of the client's situation. Provides social work services to persons 18 years and and older who are not caring for children. In addition, provides community resources and internal programs.
