Description

MEDICAL CARE: Physicians, nurse practitioners, and physician assistants provide health care services to adults and children who are uninsured, ineligible for other public health care programs like Medicaid, and have a total household income at or below 200% of the federal poverty level (FPL). Also serves adults and children who are experiencing homelessness or are recipients of Medical Assistance.

NEW PATIENTS: Must complete a Universal Client Intake Form online. A staff member will contact within 24-72 hours.

TELEHEALTH: Offered to existing patients by appointment only. Schedule via email or phone.

SUBSTANCE USE RECOVERY SERVICES: Assesses substance use, addiction history, and counseling needs. Refers to individual counseling, group counseling, the Frederick County Health Department's CORE program, and other local agencies as needed.

Income can be no more than amount listed below for the size of the family. The maximum fee charged is a $5.00 co-payment per visit, but fees can be waived due to financial hardship. There are health insurance navigators on site.

Add $10,760 for each additional person.

Households with incomes above 200% of the federal poverty level are not eligible for the Sliding Fee Discount Program, however, they may be eligible for health care services billed at full price (payment plans are available).

No one seeking health care services will be denied care due to immigration status.
