Voluntary case management program for pregnant and/or parenting teens under the age of 21 to help them:
- access appropriate medical, educational, legal and social services to develop independent living skills and become self-sufficient
- receive information on prenatal care, parenting and child development during regular home visits
- participate in occasional educational and social group activities
Coordinates panels at area schools of teen parents who talk about the hardships of being a parent at a young age to try to prevent further teen pregnancies.
Individualized support for pregnant and parenting young adults through age 24 including:
- Preparing for baby information such as prenatal care, safe sleep, feeding, bonding, diapering, and baby care- Services and support are delivered through regular home visits- Child development information to help promote healthy growth and development- Support in accessing appropriate medical, educational, legal and social services to develop independent living skills and become self-sufficient- Parenting information and support to help young parents feel confident in their parenting skills
