PERMANENT SUPPORTIVE HOUSING: Provides onsite housing or offsite subsidized housing for chronically homeless, disabled single adults who meet eligibility requirements. Also offers case management and primary health care. Families or adults with children are not eligible. Entry point for low-barrier (no requirements for sobriety or medication management) permanent housing for chronically unhoused adults.
