Provides Enhanced Care Management, which is comprehensive care management from a single lead care manager who coordinates all health and health-related care, including physical, mental, and dental care, and social services.

• Transitioning From Incarceration

Serves the following Youth Population of Focus:

• Experiencing Homelessness-Homeless Families Or Unaccompanied Children and Youth, 
• Serious Mental Health and or SUD Needs (includes children with high ACEs scores), 
• Transitioning From Incarceration
