Description
|
ADULT MEDICAL DAY CARE: Provides socialization, activities and medical supervision. Includes wheelchair accessible transportation to and from the center and medical appointments, rehabilitation, Podiatry, and Mental health services; balanced meals and snack; recreational activities, structured daily exercise, weekly bible studies; medical supervision, and assistance with personal care and daily living activities. A social worker, a registered nurse, speech and physical therapists, eye and foot doctors are on staff and are available. Services offer at a 25 acre campus that also includes amenities such as a beauty parlor, whirlpool tub, and a patio with elevated gardens.

Eligibility
|
Adults, ages 45 and up. Physically or mentally impaired or with chronic health problems requiring daytime supervision.
|
|
|
|

Program Fees
|
Accepts Medicaid, VA, private pay, and government financial assistance. Private pay is $114 per day and $117 per day effective July 1, 2024.
with a security deposit equal to one month of service is requested. Medical assistance or Medicaid wavier qualification needed individuals medically and financially qualify for services. Individuals 65 or older and Baltimore City residents may call 410-396-2273 for additional financial help. Individuals 65 or older and Baltimore County residents may call 410-887-5793 for additional financial help. Veterans that qualify for VA additional financial help will receive 2 days of care per week.

Application Process
|
Accepts referrals from physicians, hospitals, social services agencies such as Offices on Aging and DSS, and interested persons.
