Description

AGING & DISABILITY ASSISTANCE: Statewide resource provides seniors (age 60 and older), individuals with disabilities, caregivers and families with information and assistance about long term services and supports including health promotion, disease prevention, transportation, housing and income support programs so informed decisions can be made for individuals to remain at home and plan for future needs. Offers benefits screening to determine eligibility for public long term supports including level of care determinations for Medicaid nursing facility and home and community-based service waiver programs.
