Description
|
OVERVIEW: Provides services to people age 65 and older as well as individuals 18 and older with disabilities. Some services include information and assistance about long-term services and supports to allow individuals to remain at home and plan for future needs, senior centers, Ombudsman program, and a call center.
SERVICES:
1) GUARDIANSHIP PROGRAM: Provides services on behalf of individuals, ages 60 or older, who have been certified by two physicians to be incapable of caring for themselves. Guardianship is sought through a court ordered process to ensure the health and welfare of such persons when no other appropriate person is willing or able to do so.
2) LONG TERM CARE OMBUDSMAN PROGRAM: Serves residents of nursing homes and assisted living facilities. Receives, investigates, and seeks to resolve complaints of patients' rights violations.
3) MARYLAND ACCESS POINT: (see separate listing)
4) SENIOR CENTERS: (see separate listing).
5) ADULT DAY SERVICES: (see separate listing).
6) STATE HEALTH INSURANCE PROGRAM (SHIP): Provides confidential assistance for older adults and adults with disabilities on Medicare about their health insurance options.
7) HOME AND COMMUNITY BASED SERVICES: Includes Community Personal Assistance Services [CPAS], Community First Choice [CFC], and Home and Community Based Options Waiver [HCBOW]. Allows services covered by Medicaid to be offered in home (case management, home-delivered meals, and nursing services).
