Description
|
CASE MANAGEMENT: Assists people to obtain needed home and community based long-term services, equipment and programs so that they can remain in their own homes and be self-efficient and self-reporting. Prevents abuse, neglect or exploitation of vulnerable adults. Case management services include assessment, linkage to services within DSS and the community and continuous evaluation of the client's situation.
