Description
|
FAMILY PRESERVATION: Provides home-based services to families in crisis whose children are at risk of out-of-home placement. This preventative, short-term service identifies needs and strengths of families and assists families in becoming stable and connected to community services and resources. Works collaboratively with the family, including extended family members, service providers, and formal and informal supports to plan and coordinate services and/or assistance to the family. Includes reunification services to parents of children in out of home placement and substance exposed newborn (SEN) cases.
