Description FINANCIAL ASSISTANCE: Provides assistance, when funds are available, to eligible vulnerable adults. Assists with BG&E turn off notices, eviction prevention, burial assistance on a case by case basis, and purchasing a bed or other furniture. Individuals must bring proof of need and proof of income to be screened for eligibility. For assistance with furniture, individuals must bring proof of income and a statement from a furniture store (such as Lexaco) with the purchase price. Individuals can walk in for assistance Mon-Fri, 8:30AM-12:15PM and 1:30-4:15PM. Will assist individuals in public housing/Section 8. Individuals are eligible for assistance once a year to receive a grant of up to $800. 

CASE MANAGEMENT: Client needs to be a willing participant and able to consent. Provides case management services to help eligible adults to continue to live full and independent lives in the community. SSTA can secure institutional care when appropriate. Together, a case manager and the adult complete a comprehensive assessment of the adult's needs and resources to develop an individualized plan of service. Play may include the adult's family, friends, and community, as well as other established agencies. Also assists in finding other financial resources when necessary to avoid eviction/utility shut-off.

Eligibility Individuals 18 years old and older with no minor children. Do not have to have a disability for services. Applicants must meet guidelines regarding income, provide documentation of having applied for MEAP prior to inquiring about utility assistance, demonstrate proof of eviction notice. Call for more information.
