Program
Eligibility Maryland resident. Parent(s)/guardian(s) with children 5 and under, pregnant and postpartum, nursing mothers, and meet income guidelines.
Languages Offered Bilingual staff and translators on call.
Program Fees None.
Application Process Call to make an appointment. You will need to bring each person who is applying for benefits to the first appointment, including infants and all children up to age five and the required documents.
Documents Required One form of proof of household income. • Recent pay stubs, or benefit letter: SSI, Social Security, Unemployment • Medical Assistance card • Independence Card with register receipt (30 days) • Tax Return One proof of identity for each individual on the application. • Birth Certificate • Driver's License or MVA identification • Social Security Card • Picture Identification (Government, School, Employment) Proof of your address • Driver's License or MVA Identification • Official Bill or Statement • Rental or lease agreement • If applicable • Proof of pregnancy • Immunization record (infant or child) • WIC referral
