Description
|
OVERVIEW: Provides individuals 16 and older with mental, physical or developmental disabilities a daily structured socialization and health care program. Provides clients with transportation to and from center as well as 2 meals and 1 snack per day.
SERVICES:
SOCIALIZATION: Offers daily living skills and recreational activities including bowling, day trips, festivals, train rides, lunches at area restaurants, arts and crafts, bingo, card games, talent shows, and special entertainment such as singers, dancers and motivational speakers.
Home visits made for pre-admission.

Eligibility
|
Individuals ages 16 and older who are physically, mentally, or developmentally disabled and persons 55 years and older with Alzheimer's.
|
|
|
|
|
|
|
|
Program Fees
|
Private pay is $100.00 a day. Accepts Medical Assistance, long term care insurance, and persons age 55 and older may qualify for sliding fee scale.
|
