Description MEDICAL CARE: Provides low income adults and children who are uninsured or have Medicaid/Medical Assistance with needed medical care. Uninsured participants must meet income eligibility guidelines to enroll.

SERVICES:
COVID-19 TESTING: Available Monday - Thursday 9:30am-11:30am for anyone who is symptomatic and/or has had direct contact with someone who tested positive.  Must have appointment.

NEW PATIENTS:  Must complete a Universal Client Intake Form online at bit.ly/FCAAHealthIntake.  A staff member will contact within 24-72 hours.

TELEHEALTH: Offered to existing patients by appointment only.  Schedule by emailing to telehealthfcaa@cityoffrederickmd.gov or call 301-600-1393 or 301-600-1506.

Income can be no more than amount listed below for the size of the family. The maximum fee charged is a $5.00 co-payment per visit, but fees can be waived due to financial hardship. There are health insurance navigators on site.

SUBSTANCE USE RECOVERY SERVICES: Clinical assessment of addiction history and counseling needs. Medical assessment of health needs by a licensed medical professional. Individual treatment planning including referrals to individual or group outpatient counseling. Medication assisted treatment, under the supervision of licensed physician assistants for the treatment of opioid addiction. Immediate referral to the CORE program operated by the Frederick County Health Department. Will coordinate with local agencies to provide resources and other services as needed.

Eligibility Adults and children who are homeless, income eligible, or Medical Assistance recipients who are enrolled with an MCO that we participate with.
